1. Purpose of the Study: The Penn State Graduate School has asked us to document our track record in graduate education and its influence on society at all levels. Therefore, we ask you now to share with us information regarding your professional life and accomplishments.

2. Procedures to be followed: This online survey consists of 15 questions. You may cut and paste from a resume or similar source to minimize the time that this might otherwise entail. If you prefer a pencil-and-paper survey, please request such from Dr. Wayne Myers whose contact information is listed above.

3. Duration: It will take about 15 minutes to complete the survey.

4. Statement of Confidentiality: For the most part, we are asking for information that is in the public record but of which we may not be immediately aware. Publications, professional positions, and awards are of this nature. Any information that you provide that is not in the public record is confidential. We provide data in summary form to the Graduate School for institutional purposes. Pencil-and-paper data will be stored and secured in the Forest Resources Building under lock. Online data will be stored and secured in a password-protected file. No personally identifiable information beyond the public record will be shared in any publication or presentation that results from this research.

6. Voluntary Participation: Your decision to participate is voluntary. You can stop at any time. You do not have to answer any questions you do not want to answer.

You must be 18 years of age or older to take part in this survey (all bona fide graduate alumni are of eligible age). Completion and submission of the survey implies your consent to participate in this research. Please print a copy of this form for your records.
